4. The customer must be able to know what their maximum exposure ($$) would be 24x7x365 (endless co-pays do not allow for this!)
5. Price must be reasonable for the customer!
 
For your protection - If possible, you should own your own health insurance plan for you and your family.
Owning your own plan is almost always less expensive than the amount you have to pay to include your family on your company plan.
